Six special locations called Malignant Tunnels in Diablo IV appear at the end of the story content in Season 1, serving as dens of Elite enemies that offer rare rewards. Outgrowths at the ends of these Tunnels contain Malignant Hearts that you can choose between to add passive buffs for your character. Although these places seem to function like dungeons, there are some key differences to keep in mind.
Malignant Tunnels appear on the Sanctuary map marked with the same gate icon as a dungeon, but with a green leaf showing what color of Malignant Hearts can be found there. There are four types of Malignant Hearts to farm in Diablo 4 at specific Tunnels. These items act like Gem Slots, fitting into Infested openings in your gear, but you have to enact a Binding Ritual to Cage these artifacts for proper use.
Similar to dungeons, you will have to complete small objectives to progress deeper into the Tunnels, which will be filled with Elite Malignant enemies. These foes have a chance to drop Caged Hearts, which will always match the color of the Invoker you chose to activate the Tunnel. Enacting a Ritual of Binding pits you against waves of Malignant enemies that you must defeat before gaining a new Heart.
Even though you’ll be able to find Malignant Hearts from drops throughout a Tunnel, the main draw of these places comes from the Outgrowth at the end. Two different Malignant Hearts will be offered here for you to pick from, enacting another Binding Ritual when you come to a decision.
Traveling to Malignant Tunnel locations in Diablo IV gives you plenty of options to collect Hearts of all kinds if you’re willing to explore.
